Something like this.

Frozen Hamlet

Land

Tap: Add 1 to your mana pool.

1: Frozen Hamlet becomes a Plains or Island with this ability.

The only issue I have with the card is memory, and I haven't really come up with a better way to represent it. If the cards were printed there would be colored borders on half the card so each half would represent a different land type. I had come up with these before Rav block and there was an alternate cost of 2 life to the 2nd ability, but now that just makes them to similar.

You could try this for a similar effect (and solve the memory issue)

T: add 1 to your mana pool.
T: add U or W to your mana pool. Use this ability only if there is a charge counter on [this card].
2: put a charge counter on [this card].
