Do I have to pay multiple times for CoP:B to prevent damage all damage from Pestilence, or do I only have to pay once?

You have to pay 1 for every activation of pestilence.

And the same goes for Protective Sphere?

If I'm not wrong, for Protective Sphere it is a one-time effect. Once you activate it you will be protected against a certain source (Pestilence, for instance) for this turn.
